Witness — multi-node consensus

Independent attestations of entropy/pulse. Each witness Ed25519-signs ${pulse_id}|${value_hex}|${witnessed_at} over its local observation. Consensus = all reachable witnesses agree on (pulse_id, value_hex) and chain_verified=true.
