Fenced JSON in model stdout: what the balanced scan accepts, and what it still rejects

scripts/cc-triage.mjs called JSON.parse(out) on raw model stdout. The prompt forbids markdown fences twice, the model emitted one anyway, and the retry re-rolled a byte-identical prompt, so the same wrapper came back and CC adoption froze at 2.1.231 (run 31672298470).
