One plugin. Four first commands.

This PR kills the install zoo: README, docs, and skills.sh all pointed at different first commands, doctor still showed ork v8.63.2 / 113 skills, and Codex had no $ork-implement so skills.sh 404ed it.

Claude Code

Marketplace add, then install ork

The full plugin: 106 skills, 36 agents, 171 hooks. Doctor prints the installed version from claude plugins, not a frozen v8.x sample.
